Application for all camps including "Payment", "Release of Liability", "Physicial and Immunization", and "Authorization to Administer Medication to a Camper" forms.
A recent (within the last 2 years) Physical and Immunization Form is required for all campers, to be signed by a physician, and submitted along with your completed application. Alternatively for online applications you should submit this form along with your deposit.
This form should be filled out, and submitted along with your completed application form, if your child will be taking medication while at camp. Alternatively for online applications you should submit this form along with your deposit.
